The following forms will help you obtain a passport, mail in your ballot, get a trade name certificate, and other Clerk’s Office functions.

Passports

Please note: The U.S. Department of State requires that all passport forms must be printed in full size on 8 1/2″ x 11″ paper without any information being cut off. Otherwise, a new form will have to be completed.

  • DS-11 (In-person application for new or renewal passports)
  • DS-82 (Renewal by mail application)
  • DS-3053 (Parental consent form for when one parent cannot be present, must be notarized)
  • DS-64 (Statement regarding a lost or stolen passport)

Primary Election Petitions

PLEASE NOTE: The single signature sheets are intended to supplement the regular petitions found below. The candidate information on the first page as well as the Certificate of Acceptance and Oath of Allegiance within the regular petition must be completed. Circulators must still fill in their information if any in-person signatures were collected by them. Petitions must be filed with the appropriate Municipal or County Clerk by Monday, April 5th, at 4:00pm, however they may be submitted electronically instead of in-person. Please contact their office for more information.
